Illness and Hospital Stay

Crisis Intervention - Addressing issues related to child maltreatment, domestic violence, and elder
abuse.
Pre-Admission and
Discharge Planning
Early
Intervention Program
- Provides CORE and Supplemental evaluations for infants and toddlers, ages 0-3, with suspected disabilities or delays
Patient Support Groups
Transportation - For
clinic patients with Medicaid coverage
