Laser Safety Self-Assessment Test

Instructions: This test is completely electronic. No hard copies will be accepted. A minimum grade of 75% is required to achieve a passing score.

1. In order to complete the test online, your PC or Mac must have Adobe Acrobat Reader version 8 or higher.
2. Click on the link below to open the PDF document. You will be able to enter your answers directly within the form.
3. When you are ready to digitally sign the document, click on the box next to "Physician's Signature" and follow the instructions for creating a new self-signed PKCS #12 digital ID file. Need assistance with digital signatures?
4. When you are ready to submit the test, click the green "Submit Form" button at the end of the form. If you are using Outlook, you'll choose "Desktop Email Application" otherwise you will use the "Internet email" option (for other email programs, i.e. AOL, Hotmail, Gmail, Yahoo, etc.).
5. Once you submit the form, it will be emailed to Arthur Tortorelli for grading.
