2005 Community Service Plan
The New York Eye and Ear Infirmary's Community Service Plan (CSP) is updated and submitted annually to the NYS Department of health. The CSP also includes our Mission and Vision statement, our community outreach programs and it describes the approximately three dozen community affiliations and agencies with whom we worked in the previous year.
